Students may request a leave of absence from the College of Osteopathic Medicine for personal or health reasons.   

To request a leave of absence, the student must first meet with an advisor from the Office of Academic & Career Advising to discuss intent to take a leave of absence.  Should a student decide to move forward with their request, the advisor will instruct students to complete the Leave of Absence request form.

• Extension Policy (Academic and Personal)

To discontinue from Michigan State University College of Osteopathic Medicine D.O. program, a student must complete this form and attach a personal statement.  Once received, the MSUCOM Registrar will reach out to discuss discontinuing from the program.  Please note, you will not be processed until the preceding steps take place.  If you have additional questions please email: com.osteomedreg@msu.edu.
