Address Change 

Students must have a current address and phone number on file with the college and the university.

• To change your address or phone number with the university, visit the MSU Office of the Registrar.
• To change your address or phone number with the college please visit the Student Portal.  

Name Change

Bring a legal document, driver's license, voter registration, social security card, passport, or marriage license to the Michigan State University Office of the Registrar.

To order a duplicate or replacement diploma, complete the Diploma Order Form and submit as instructed to the MSU Office of the Registrar.

Official Transcripts

Current students and graduates of MSU College of Osteopathic Medicine

Requests for official transcripts must be submitted to the MSU Registrar’s Office directly. Complete online Transcript Request, or send written request to:

General Transcript Requests for Residency and Fellowship applicants

Follow the instructions for requesting a transcript through the University Registrar's Office.

You will want to electronically send the transcript to the Academic and Career Advising team. Please email com.acadvising@msu.edu with a subject - Attention: Residency Application

Once the transcript arrives to the advising team, it will be uploaded to ERAS or added to your appropriate application (SF, AUA, Military, CaRMS).

Clerkship students and VSLO

If a transcript is required, students will need to order their MSU transcript and can have electronic copies sent to COM.Clerkship@msu.edu. Transcripts will be automatically uploaded to VSLO once received by the Clerkship team.

Students can also order electronic copies of their transcripts and have them emailed directly to the program/requestor.

Tip: Make sure your NGR or ET grades are updated prior to requesting your transcript. Please contact the Course Assistant listed on the first page of each course syllabus to clear up any NGR or ET grades.

Unofficial Transcripts 

Current students interested in viewing or printing their grade report can do so by logging into the Student Portal or StuInfo.

Entering Class Students

Students who may need to submit an updated copy of their official transcript showing completion of requirements or degrees since application have two options.

Official electronic transcripts may be sent to transcript@com.msu.edu Official paper transcripts may be sent to:
